The proposed resolution authorizes replacing the name Heartland Health Outreach, Inc. with Heartland Alliance Health in adopted resolutions and executed Purchase of Service contracts with the City of Madison. Adopted RES-17-00909 (File ID 49339) awarded $65,000 to Heartland Health Outreach, Inc. to supplement supportive services at 715 Rethke (Rethke Terrace Appartments). This name change is administrative only and does not alter the scope of the projects previously approved. No additional City appropriation is required.

Background:
The City of Madison Community Development Division has been notified that Heartland Health Outreach, Inc, has changed the legal name of the organization to Heartland Alliance Health. The purpose of the name change is to address confusion with participants, donors and within the service sector.

On November 21, 2017, the Common Council authorized the allocation of $65,000 to Heartland Health Outreach, Inc to supplement supportive services at 715 Rethke (Rethke Terrace Apartments) - (RES-17-00909, Legistar #49339). An amendment is required to the executed Purchase of Service contract with Heartland Health Outreach, Inc to reflect the new legal entity, Heartland Alliance Health.

Furthermore, Heartland Housing and Heartland Health Outreach, Inc submitted a proposal, in response to the City’s Request for Qualifications, and was selected to provide services relating to the development, property management and provision of supportive services for housing homeless adults for the project being developed on Park Street. This project will be the third phase in the City’s development of Permanent Supportive Housing.
