Your position as a City of Madison board, commission or committee member requires you to file a Statement of Interests form with the City Clerk’s Office each year. The deadline for filing 2022 Statement of Interests form was January 4, 2022.

If you have already submitted your form and are receiving this email, please contact the City Clerk’s Office by phone at 608-266-4601 or by email at clerk@cityofmadison.com <mailto:clerk@cityofmadison.com>.

If you do not have access to a computer, we invite you to visit the City Clerk’s Office and use the dedicated workstation to file. The City Clerk’s Office is open Monday - Friday from 8:00 a.m. to 4:30 p.m.  Computers are also available at all City of Madison Public Library locations.

Pursuant to Section 3.35(9)(i)2 of the Madison General Ordinances, upon receipt of this notice and until you have submitted the Statement of Interests form, you will not be able to participate in or vote on any matter before the body, but you may attend committee meetings and count toward quorum if necessary to conduct business.

The City Clerk’s Office will soon provide the Common Council with a list of committee members who have not filed. Section 3.35(9)(i)3 of the Madison General Ordinances authorizes the Common Council to revoke the appointment of any board, commission or committee member who does not file the form in a timely manner.
