Public Health - Madison and Dane County (PHMDC) proposes accepting funding from Wildwood Family Clinic, S.C. for the Wisconsin Well Woman Program (WWWP) to support case management of WWWP clients through PHMDC. Specifically, PHMDC proposed accepting $505.00 to allow WWWP to serve more people in Dane County that have breast or cervical health concerns, specifically by providing transportation services for WWWP clients to attend medical appointments.
Under the terms of the merger agreement, the Board of Health for Madison and Dane county may accept grant funds with the approval of the Board of Health for Madison and Dane County (IGA, VII, F, 1-3).
NOW THEREFORE BE IT RESOLVED that the Board of Health for Madison and Dane County, on behalf of Public Health-Madison and Dane County authorizes the Director of Public Health to accept $505.00 from Wildwood Family Clinic, S.C. to support the Wisconsin Well Woman Program.
