Board of Health Resolution #2019-29 Authorization of Budget Transfer of Preventive Health and Health Services Block Grant Funds

Public Health-Madison and Dane County (PHMDC) was previously awarded the Preventive Health and Health Services (PHHS) Block Grant from the Wisconsin Department of Health Services (DHS) on behalf of the Division of Public Health to support the Community Health Assessment process. DHS offered additional funds, $2,750, be added to the PHHS Block Grant allocation to send staff to the Journey of Facilitation training.
Funding concluded on September 30, 2019 and PHMDC is requesting approval to accept the additional $2,750 added to the PHHS Block Grant to reimburse agency expenses to send staff to the Journey of Facilitation training.
NOW THEREFORE BE IT RESOLVED that the Board of Health for Madison and Dane County, on behalf of Public Health-Madison and Dane County, authorizes the Director of Public Health, or their designee, to enter into an agreement with the Wisconsin Department of Health Services and to accept funding outlined in the agreement.
