Public Health-Madison and Dane County (PHMDC) proposes entering into a contract for purchase of services with Creative Marketing Resources. The funding for this contract is from the Wisconsin Department of Health Services on behalf of the Division of Public Health to support the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) program outreach efforts.
NOW THEREFORE BE IT RESOLVED, that the Board of Health for Madison Dane County, on behalf of Public Health-Madison and Dane County, authorizes the Director of Public Health to enter into a contract for purchase of services with Creative Marketing Resources for outreach for the Special Supplemental Nutrition Program for Women, Infants, and Children Program. Contract amount not to exceed $25,197.00.
