Madison Parks staff proposes the following conditions for the Board of Park Commissioner’s consideration:
1. THE APPLICANT FOR A PARK EVENT PERMIT SHALL AGREE TO INDEMNIFY, DEFEND, AND HOLD THE CITY AND ITS EMPLOYEES AND AGENTS HARMLESS AGAINST ALL CLAIMS, LIABILITY, LOSS, DAMAGE, OR EXPENSE INCURRED BY THE CITY ON ACCOUNT OF ANY INJURY TO OR DEATH OF ANY PERSON OR ANY DAMAGE TO PROPERTY CAUSED BY OR RESULTING FROM THE ACTIVITIES FOR WHICH THE PERMIT IS GRANTED.
2. Organizer must notify area alder at least 15 days prior to the installation of Ella Phantzpiral. Provide the alder with relevant information, including: location, date, schedule, etc. The alder to notify for the installation is

3. Parks Superintendent shall provide the final approval on the exact statue location in the park.
4. A certificate of insurance naming the City of Madison as an additional insured shall be provided no less than one week prior to installation.
5. Organizer will contact the Central Parks Maintenance Supervisor, Chad Hughes, at cmhughes@cityofmadison.com at least 7 days prior to installation to review site plans. Organizer must abide by all recommendations/stipulations provided by the Maintenance Supervisor.
6. If any staking is required for the installation, People for the Ethical Treatment of Animals (PETA) is responsible for contacting Digger’s Hotline, 811 or 800-242-8511, 7-10 days prior to event and providing the Parks Office with the ticket number.
7. All fees will be paid and deposits made at least one week before the event. We accept cash, check, or charge (VISA or MasterCard). To make a credit card payment via phone, please call (608) 264-9289.
