The proposed resolution authorizes the renewal of an agreement with Unity Point Health - Meriter under which the Fire Department will provide Paramedic Case Management services for approximately ten clients during the period of January 1, 2018 through December 31, 2018. Costs associated with providing these services are $70,000 and will be funded by Unity Point Health - Meriter (Munis project #17503). There is no impact on the levy.

Body
The Madison Fire Department entered a Memorandum of Understanding with Unity Point Health - Meriter in February 2016 to provide paramedic case management services. The MOU requires the Madison Fire Department to provide 20 hours weekly of Paramedic Case Management for approximately 10 clients that will be jointly identified by Meriter and the Madison Fire Department. The Madison Fire Department will provide follow-up with the identified clients and work with the client to connect them to existing community resources. Unity-Point Meriter has identified funding of $70,000 for this program for 2018, all other provisions of the original MOU will remain in place.

WHEREAS, the Madison Fire Department has trained Mobile Integrated Health Paramedics; and,

WHEREAS, Unity Point Health - Meriter wishes to have the Madison Fire Department provide Paramedic Case Management for mutually agreed upon Madison residents; and,

WHEREAS, Unity Point Health - Meriter has proposed to renew the MOU from Jan 1, 2018 - December 31, 2018 that may be extended or renewed for successive one year periods upon written agreement of the parties; and,

WHEREAS, Unity Point Health - Meriter has agreed to pay $70,000 for this service for the 2018 term,
