Madison Fire Department (MFD) will provide 20 hours weekly of Paramedic Case Management for approximately 10 clients that will be jointly identified by Meriter and MFD. Unity Point Health - Meriter will pay MFD $50,000 for these services which will cover personnel expense ($42,000) and related software ($8,000).

Funding and costs associated with this MOU will be accounted for in Munis project 17462.

PREAMBLE
The Madison Fire Department has developed a Mobile Integrated Health Team in 2015 as part of the EMS Division within the fire department. This team works to address at risk populations and high service utilizers of emergency medical services. Through a subaward agreement in 2015 with the University of Wisconsin Board Regents, the Madison Fire Department has trained a limited number of department members on transitional coaching methods. The goal of these coaching techniques is to work with members of our community on various options for addressing their medical needs that do not require emergency transport or emergency treatment through an emergency department admission.

The MOU requires the Madison Fire Department to provide 20 hours weekly of Paramedic Case Management for approximately 10 clients that will be jointly identified by Meriter and the Madison Fire Department. The Madison Fire Department will provide follow-up with the identified clients and work with the client to connect them to existing community resources.
