PREAMBLE
On March 10, 2011 the Community Development Authority (the “CDA”) of the City of Madison adopted Resolution No. 3010 (Legistar File ID# 21614) that authorized the award and execution of a contract with J.H. Findorff & Son, Inc. (the “Contract”) for the build-out of space within the Village on Park that will be leased to Public Health-Madison and Dane County. Subsequent to the execution of the Contract, Supplemental Instructions to the contractor were issued for the replacement of the store front window of the former South Madison Branch Library space which is part of the Leased Premises. This will increase costs above the Contract amount. This resolution will authorize a change order to the Contract to cover the increased costs associated with replacing the window.
NOW THEREFORE BE IT RESOLVED that the CDA hereby authorizes a change order in the amount of $15,572 to the contract executed with Findorff & Son, Inc for the increased costs associated with replacing the store front window in the former South Madison Branch Library space in The Village on Park that will be leased to Public Health-Madison and Dane County.
BE IT FURTHER RESOLVED that the Chair and Executive Director and Secretary of the CDA are hereby authorized to execute, deliver and record such documents and to take such other actions as shall be necessary or desirable to accomplish the purposes of this resolution.
