Fiscal Note
The Addendum to Employment Services Agreement provides for an annual salary of $140,004.80, plus benefits. This amount will be provided for within the 2011 Public Health Executive Operating Budget. The Public Health Director is an employee of Dane County, with costs of the position shared with the City on a 55.378% County / 44.622% City basis.

Confirming the renewed appointment of Thomas L. Schlenker, M.D. to the position of Unified Public Health Director subject to the execution of an Addendum to the Employment Services Agreement with Dane County.
Body
Whereas Mayor Cieslewicz, County Executive Falk, and the Joint Board of Health appointed Thomas L. Schlenker, M.D. to the position of Unified Public Health Director on December 19, 2005 for a five (5) year term; and
Whereas; the parties wish to renew the appointment for an additional five (5) year term and have reached agreement on the terms and conditions of employment;
Now, therefore, be it resolved that Dr. Schlenker's appointment to the position of Unified Public Health Director is hereby confirmed and the City agrees that the County Executive may execute an addendum to the employment services contract with Dr. Schlenker to serve as Director of Public Health for Madison and Dane County for an additional period of five (5) years.
