WHEREAS, Churchill Crossing, LLC has satisfactorily completed the installation of sanitary sewer, storm sewer, and street and terrace improvements by private contract, in accordance with the City of Madison Standard Specifications for Public Works Construction, in: Churchill Crossing PUD / 615 Pleasant View Road CSM (9th AD)


NOW, THEREFORE, BE IT RESOLVED

1. That said sanitary sewer, storm sewer, and street and terrace improvements be and are hereby accepted.

2. That said sanitary sewer improvements are hereby assigned to the Madison Sewer Utility for ownership and maintenance and that the attached Schedule "A" representing the actual cost of construction of these improvements shall be reflected in the Madison Sewer Utility's "Utility Plant In Service".

SANITARY SEWER ACCOUNTING DATA

A) Total Contract Value (including Engineering
and Miscellaneous Costs) $ 20,532.60

B) Contributions in Aid of Construction:
a) Public Sewer Components $ 20,532.60
b) Sewer Laterals $ -0-_

C) Investments in Capital (MSU Share) $ -0-

D) Total Plant Value $ 20,532.60

3. That said storm sewer improvements are hereby assigned to the Madison Storm Water Utility for ownership and maintenance and that the attached Schedule "A" representing the actual cost of construction of these improvements shall be reflected in the Madison Storm Water Utility's "Utility Plant In Service".


Increase in Total Plant Value per Schedule “A” $ 7,612.00


4. That the street, terrace and bikepath, on the following streets be and are hereby accepted:

· Old Sauk Road from Pleasant View Rd to the E line of Lot 1 CSM 12463
· Pleasant View Road from Old Sauk Rd to S line of Lot 1 CSM 12463
· Bikepath from Pleasant View Road to Shawn Trail
