Authorization to Amend the Health Dane Collaborative Memorandum of Understanding (MOU) Agreement for the Community Health Data Platform Project (2026 BOH RES-006)

Public Health Madison and Dane County (Public Health) seek authorization to amend, the Healthy Dane Collaborative Memorandum of Understanding (MOU) agreement with the members of the Healthy Dane Collaborative to continue the Community Health Data Platform.
Public Health is currently a member of Healthy Dane, a community collaborative comprised of Public Health, UnityPoint Health - Meriter, Stoughton Hospital Association, SSM Health St. Mary’s Hospital - Madison, the University of Wisconsin Hospitals and Clinics. The purpose of the collaborative is to work in tandem to create each system’s Community Health Needs Assessment and Public Health’s Community Needs Assessment.
In 2012, the members of the Healthy Dane Collaborative entered into a Memorandum of Understanding to establish a payment structure to fund a Community Health Needs Assessment Data System and contract with Conduent Healthy Communities Corporation (formerly HCI). The data is currently maintained on the following web domain:
. Public Health is responsible for administering the contract with a data platform vendor and collecting a portion of platform subscription fees from each partner.
PHMDC will provide in-kind support to the management of the data platform by assigning an epidemiologist to maintain the data platform in alignment with Collaborative vision. This support may include entering, contextualizing, and organizing Dane County population health data. The data platform will serve as a tool for community health improvement planning for both the health systems and public health.
