Board of Health Resolution #2019-24 Authorization of Budget Transfer of Preventive Health and Health Services Block Grant Funds

Public Health-Madison and Dane County (PHMDC) was previously awarded the Preventive Health and Health Services (PHHS) Block Grant from the Wisconsin Department of Health Services on behalf of the Division of Public Health to support the Community Health Assessment process. Due to the timeline of spending the grant funding, and the needs to support community engagement activities, it is being proposed to move a portion of the grant funding from purchased services to supplies.
The transfer of funding within the PHHS grant will address the following grant objectives:
• Conduct community engagement activities, outreach, and focus group sessions to collect qualitative data for the Community Health Assessment.
NOW THEREFORE BE IT RESOLVED that the Board of Health for Madison and Dane County, on behalf of Public Health-Madison and Dane County, authorizes the transfer of the amounts of the Preventive Health and Health Services Grant as outlined below, for the purpose of community engagement activities and survey collection.
