Authorizing the Madison Department of Public Health to accept a grant for consolidated contracts from the State Department of Health and Family Services-Division of Public Health for up to $292,310 in 2006 for programs for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention and maternal and child health; and authorizing the Mayor and City Clerk to sign the "Consolidated Contract Agreement" with DHFS.
Body
WHEREAS funds totaling $292,310 are available through the Wisconsin Department of Health and Family Services (DHFS), Division of Public Health, for programs for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention and maternal and child health for the period of January 1, 2006 to December 31, 2006; and
WHEREAS the Madison Department of Public Health has ongoing programs in these areas;
WHEREAS the State DHFS/Division of Public Health has consolidated its funding for these programs into a single contract to be executed between DHFS and local health departments; and
WHEREAS, the Consolidated Contract Agreement sets forth the terms by which the City is to accept the grant and the services to be provided by the Madison Department of Public Health for the period of January 1, 2006 through December 31, 2006;
NOW THEREFORE BE IT RESOLVED the City is authorized to accept a grant from the State of Wisconsin for up to $292,310 in 2006, for the above-described purposes, and the Mayor and City Clerk are authorized to sign the "Consolidated Contract Agreement" with the State of Wisconsin, Department of Health and Family Services, Division of Public Health for January 1, 2006 through December 31, 2006, for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention and maternal and child health.
