The proposed resolution authorizes an amendment to a contract between the City of Madison and Allied Wellness Center to extend the contract term to December 31, 2023. In April 2022, the Common Council adopted a resolution (RES-22-00312) to enter into a service contract for $55,541 with Allied Wellness Center effective October 2021 to September 2023 to provide services for the implementation of a COVID-19 Education Program. To date Allied Wellness Center has been paid $48,907, leaving a balance of $6,634. The contract is funded by a US EPA Environmental Justice Grant. The resolution also authorizes the extension to a later date should subsequent extensions be authorized by US EPA. No appropriation is required.

WHEREAS in 2021 the City of Madison was awarded a grant through the US EPA’s State Environmental Justice Cooperative Agreement (SEJCA) grant program in the amount of $200,000 to implement a Citywide COVID-19 Education Program; and
WHEREAS on October, 19, 2021, the Madison Common Council adopted a Resolution (RES-21-00715) to accept the award from the US EPA and amend the Sustainability Improvements Capital Budget to add $200,000 in grant funding; and
WHEREAS on April 19, 2022, the Madison Common Council adopted Resolution (RES-22-00312) to enter into a $55,541 service contract effective October 2021 to September 2023 with Allied Wellness Center to provide services for the City of Madison - Citywide Implementation of COVID-19 Education Program that is being funded through a US EPA Environmental Justice Grant; and
WHEREAS, the performance period of the City of Madison's US EPA’s State Environmental Justice Cooperative Agreement (SEJCA) grant has been extended through 9/30/2024; and
