BOH RESOLUTION #2018 - 19 AUTHORIZATION TO MODIFY TERMS OF MEMORANDUM OF UNDERSTANDING WITH THE HEALTHY DANE COLLABORATIVE PARTNERS

Public Health-Madison and Dane County (PHMDC) is a member of Healthy Dane, a community collaborative comprised of PHMDC, UnityPoint Health - Meriter, Stoughton Hospital, SSM Health St. Mary’s Hospital - Madison, the University of Wisconsin Hospitals and Clinics. The purpose of the collaborative is to work in tandem to create each system’s Community Health Needs Assessment.
In 2012, the members of the Healthy Dane Collaborative entered into a Memorandum of Understanding (MOU) to establish a payment structure to fund a Community Health Needs Assessment Data System and contract with Conduent Healthy Communities Corporation. (The data is maintained on the following web domain: <http://www.healthydane.org>). PHMDC is responsible for administering the contract and the fee schedule is as follows:
• Meriter - $8,666.67 annually
• St. Mary’s - $8,666.67 annually
• UWHCA - $8,666.67 annually
• Stoughton Hospital - $2,500 annually
The original MOU, signed in 2012, was valid for 3 years with 1-year renewals occurring annually thereafter. PHMDC and the members of the Healthy Dane propose amending the current MOU renewal terms from annual renewals to a 3-year renewal term, which would better align with the work of the collaborative and the 3-year CHNA cycle. Amending the current MOU to align with the CHNA cycle includes changing the current renewal term to an effective date of April 6, 2018 and continuing to April 6, 2021.
NOW THEREFORE BE IT RESOLVED that the Board of Health for Madison and Dane County, authorizes the Director of Public Health-Madison and Dane County to sign an amended Memorandum of Understanding with the members of the Healthy Dane to extend the renewal period.
