Board of Health for Madison and Dane County Resolution #2025-05 Authorization to Accept Additional Funds from the Wisconsin Department of Health Services for the Community Health Improvement Plan

Public Health Madison and Dane County (Public Health) proposes the acceptance of an additional $868 from the Wisconsin Department of Health Services (DHS) for work to advance the current cycle of our Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).
The Board of Health for Madison and Dane County previously approved Resolution #2023-36 authorizing the acceptance of $33,928 from DHS for the last cycle of the CHA/CHIP. PHMDC received an additional $868 from DHS, increasing the total amount to $34,796.
This money comes from the CDC’s Preventive Health & Health Services grant, which is then allocated to all local health departments in Wisconsin through DHS. These funds will be used towards projects and work that advance the goals and actions of our current CHIP. Current CHIP priority areas include Reproductive Justice, Mental Health & Substance Use, Injury & Safety, and Chronic Conditions.
NOW THEREFORE BE IT RESOLVED, that the Board of Health for Madison and Dane County on behalf of Public Health Madison and Dane County authorizes the Director of Public Health, or their designee, to sign the agreement, accept and expend the funding, and amend the 2024 Public Health Budget by $868.
