PREAMBLE

The resolution authorizing Public Health-Madison and Dane County to enter into an agreement with Madison Dental Initiative, Inc. contained a typographic error. The amount for the initial term of the contract was initially stated as $1,750, whereas it should have been stated as $3,750.

WHEREAS Resolution RES-14-00794 adopted by Common Council on November 18, 2014 (Legistar #35838) authorizing Public Health-Madison and Dane County to enter into an agreement for dental services with Madison Dental Initiative, Inc. contained a typographic error,

NOW THEREFORE BE IT RESOLVED THAT the amount for the initial term of November 15, 2014 through December 31, 2015 is corrected from $1,750 to $3,750 and the amendment to PHMDC's 2014 Adopted Operating Budget is corrected to reflect this amount.
