Expenditures and revenue are included in the Public Health operating budget. There is no anticipated impact upon cost controls or the tax levy. No additional appropriation is required.

Authorizing the Madison Department of Public Health to accept a grant from the Children's Health Alliance of Wisconsin for $4,133 for Halloween Sealant Day 2006.
Body
WHEREAS the Madison Department of Public Health has conducted the Halloween Sealant Day program targeting school-age children for the past nine years;
WHEREAS the Wisconsin Seal-A-Smile program of the Children's Health Alliance of Wisconsin and Wisconsin Department of Public Health has awarded the Madison Department of Public Health a grant to conduct the Halloween Sealant Day 2006 Program;
WHEREAS the Halloween Sealant Day 2006 will provide dental sealants to families for children ages 6 to 18 who do not have dental insurance or the financial means to obtain preventive dental care;
WHEREAS this program will be conducted collaboratively with Madison Area Technical College (MATC) and the Dane County Division of Public Health and will have a volunteer staff that include dentists, dental hygienists, dental assistants and dental hygiene students;
NOW THEREFORE BE IT RESOLVED that the Mayor and City Clerk, on behalf of the Madison Department of Public Health are authorized to sign an agreement with the Children's Health Alliance of Wisconsin for the period July 1, 2006 to June 15, 2007, 2007 for the purpose of conducting the Halloween Sealant Day 2006 program.
