Authorizing Public Health-Madison and Dane County to accept a grant for consolidated contracts from the State of Wisconsin Department of Health and Family Services Division of Public Health for up to $291,266 for programs for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention, and maternal and child health and authorizing the Mayor and City Clerk to sign the Consolidated Contract Agreement.

WHEREAS funding totaling $291,266 are available through the State of Wisconsin Department of Health and Family Division of Public Health for programs for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention, and maternal and child health for the period of January 1, 2007 through December 31, 2007; and
WHEREAS Public Health-Madison and Dane County has ongoing programs in these areas;
WHEREAS, the State of Wisconsin Department of Health and Family Services Division of Public Health has consolidated its funding for these programs into a single contract to be executed between the Department of Health and Family Services and local health departments, and
WHEREAS the Consolidated Contract Agreement sets forth the terms by which the City of Madison is to accept the grant, as well as the services provided by Public Health-Madison and Dane County (Madison Department of Public Health) for the period of January 1, 2007 through December 31, 2007,
NOW THEREFORE BE IT RESOLVED that the City of Madison is authorized to accept a grant on behalf of Public Health-Madison and Dane County (Madison Department of Public Health) from the State of Wisconsin for up to $291,266 for emergency preparedness, childhood lead poisoning prevention, immunizations, prevention, and maternal and child health, and the Mayor and City Clerk are authorized to sign t...
