Title
Authorizing a representative of the Human Resources Department to sign a contract with Home Health United to provide flu immunization clinics for City employees on City property.
Body
WHEREAS, the trial influenza immunization program conducted in the fall of 2003 with Home Health United was well appreciated and extensively utilized by City workers and well run; and
 
WHEREAS, Home Health United is willing to conduct a similar immunization program for City workers and four different work locations; and
 
WHEREAS, there will be no cost to the City (an employee shall pay the cost of the flu shot if s/he has no HMO, otherwise, Home Health United has agreements with all of the HMO's and the Standard Plan for reimbursement for immunizations to covered employees);
 
NOW THEREFORE BE IT RESOLVED that the Human Resources Director is authorized to execute Home Health United contracts and forms necessary to hold influenza immunization clinics at City locations for the benefit of City employees and their family members to the extent authorized by their respective HMO or at Home Health United's fee per immunization paid in advance;
 
BE IT FURTHER RESOLVED that City staff may be used to help with the administrative details such as collecting consent forms, confirming employee information and assisting with people flow;
 
BE IT FURTHER RESOLVED that the City Attorney's Office shall review all Home Health United contracts as to form.
