Authorizing an Amendment to the Agreement with the University of Wisconsin Medical Foundation for Public Health Madison & Dane County Medical Director Services (2026 BOH RES-043)

WHEREAS, the Board of Health for Madison and Dane County on behalf of Public Health for Madison & Dane County (Public Health) previously authorized an agreement with the University of Wisconsin Medical Foundation, Inc., acting on behalf of the faculty physicians in the Department of Family Medicine and Community Health of the University of Wisconsin School of Medicine and Public Health at the University of Wisconsin-Madison, for Public Health Medical Director Services provided by Jonathan Temte, MD; and
WHEREAS, pursuant to the existing agreement, Jessica Dalby, MD was previously approved by Public Health Madison & Dane County as a UW Faculty Physician authorized to provide Medical Director Services in the event of Jonathan Temte, MD's unavailability; and
WHEREAS, Jonathan Temte, MD will retire from his role as Medical Director effective October 31, 2026; and
WHEREAS, effective November 1, 2026, Jessica Dalby, MD will assume the duties and responsibilities of Medical Director under the agreement; and
WHEREAS, the parties desire to amend the agreement to designate Jessica Dalby, MD as the physician providing Medical Director Services; and
WHEREAS, the parties have determined that a temporary increase in service hours and compensation is appropriate during the transition period of November 1, 2026 through June 30, 2027 to support onboarding and orientation to the department's programs, operations, and responsibilities as Medical Director; and
NOW, THEREFORE, BE IT RESOLVED that the Board of Health for Madison and Dane County, on behalf of Public Health Madison & Dane County, authorizes the Director of Public Health, or their designee, to execute Amendment No. 1 to the agreement with the University of Wisconsin Medical Foundation, Inc., acting on behalf of the faculty physicians in the Department of Family Medicine and Community Health of the University of Wisconsin School of Medicine and Public Health at the University of Wisconsin-Madison, designating Jessica Dalby, MD as the UW Faculty Physician providing Medical Director Services effective November 1, 2026; and
BE IT FURTHER RESOLVED that Amendment No. 1 shall remove the provision identifying Jessica Dalby, MD as an alternate UW Faculty Physician authorized to provide Medical Director Services during the unavailability of Jonathan Temte, MD, and shall provide that Medical Director Services may be provided by another UW Faculty Physician approved by Public Health Madison & Dane County in the event Jessica Dalby, MD is unavailable; and
BE IT FINALLY RESOLVED that Amendment No. 1 shall revise the service hours and compensation provisions for the period of November 1, 2026, through June 30, 2027, to support the transition of Jessica Dalby, MD into the Medical Director role, with all other terms and conditions of the agreement remaining unchanged.
