Body
The Wisconsin Well Woman Program in Dane County was coordinated for the last twelve years through the Dane County Division of Public Health and now by Public Health-Madison and Dane County. Well Woman provides free breast and cervical screenings for eligible women who are ages 35-64, uninsured or underinsured and who are 250% of the Federal Poverty Level. These screenings, provided by a network of 25 local hospitals and clinics, include breast self-exam instruction, clinical breast exams, mammograms and breast diagnostic services. Well Woman is funded by the Centers for Disease Control and Prevention--Breast and Cervical Cancer Early Detection Program.  Well Woman does not have the resources to cover treatment for breast cancer.  
  
Since 2000, the Susan G. Komen Breast Cancer Foundation has funded a Treatment Access Fund for women. The fund provides breast cancer screening, diagnosis and treatment services for uninsured women in Dane County who do not qualify for any financial assistance program and do not have any form of health coverage. An eligible woman may submit her medical bills for breast related services. The program has negotiated with most health care systems in Dane County to accept a 50 percent payment on these bills. Based on available funds, breast related medical expenses, for eligible women, are paid directly to the healthcare provider.  Since 2000, the Treatment Access Fund has assisted approximately 130 women and paid more than $75,000 toward medical bills.
 
In 2007, 625 women were enrolled and screened through Well Woman in Dane County and twelve women were diagnosed with breast cancer with funding  provided for their screening and diagnosis. Ten of these women were then enrolled in the state's WI Well Woman Medicaid to cover the cost of treatment.  
WHEREAS the Dane County Division of Public Health has coordinated the Wisconsin Well Woman Program in Dane County for twelve years and this will now be coordinated by Public Health-Madison and Dane County,
 
WHEREAS the Susan G. Komen Breast Cancer Foundation is providing funding for a Treatment Access Fund,
 
NOW THEREFORE BE IT RESOLVED that the Board of Health for Madison and Dane County, on behalf of Public Health-Madison and Dane County is authorized to enter into an agreement with the Susan G. Komen Breast Cancer Foundation for $49,577 for the Treatment Access Fund that is part of the Wisconsin Well Woman Program in Dane County for the period April 1, 2008 to March 31, 2009 and the Director is authorized to sign the agreement.
 
BE IT FURTHER RESOLVED that the 2008 Adopted Operating Budget for Public Health-Madison and Dane County will be revised as follows:
