Public Health Madison & Dane County (Public Health) seeks to enter into Memorandum of Understanding (MOU) agreements with Health Maintenance Organizations (HMO) for the coordination of client care.

Public Health is a local health department which provides many public health services, including the provision of services to Medicaid recipients. The Wisconsin Department of Health Services requires a written MOU between local health departments and state contracted HMOs to provide a framework for effective communication, outline procedures for sharing information, and outline respective roles and responsibilities. This includes MOUs for specific services including Women, Infant and Children (WIC) and Prenatal Care Coordination (PNCC), as well as additional services provided by Public Health.

Public Health is seeking authorization to execute an MOU with each of the health maintenance organizations listed that operate in Dane County below:

· Anthem Blue Cross and Blue Shield
· Chorus Community Health Plans
· Dean Health Plan
· Group Health Cooperative of South Central Wisconsin
· Independent Care Health Plan (iCare)
· MHS Health Wisconsin
· Molina Health Care
· My Choice Wisconsin
· Network Health Plan
· Quartz
· UnitedHealthcare of Wisconsin

Each MOU becomes effective on the date that both the HMO and PNCC provider have signed it and will remain in effect until terminated by either party.

NOW THEREFORE BE IT BE RESOLVED, that the Board of Health for Madison and Dane County authorizes Public Health Madison & Dane County to enter into Memorandum of Understanding agreements with each of the above named Health Maintenance Organizations for the purposes described above; and
